Summit HIRE Act Update - If you have questions regarding the new HIRE act, this is a great place to start. Click on the link to download a document that captures important information for employers.
W-11, HIRE Act Affidavit - If you have an employee that qualifies for the HIRE act, please download this form and fill out.
SS-4 - If you are a new employer and you need to get an Employer Identification Number, please click on this link to download the document for an application.
Tax Information Authorization - IRS form 8821. Employers should download this form and fill it out so that Summit Payroll can discuss matters with the IRS regarding tax payments and filings.
Reporting Agent Authorization - IRS form 8655. Employers should download this form and fill it out so that Summit Payroll can file returns on behalf of the client
W-4 - Employees can download this form and return to employer to change federal withholding information.
I-9 - Employers should have employee verification done upon hiring and keep for the employee’s file.
DE-1 - The DE-1 is the application for an employer number with the Employer Development Department of California. Download and fill out this form if you have not applied for a state identification number and you intend to be an employer in the state of California.
DE-4 - Employees can download this form and return to employer to change state withholding information for the state of California.
State Power of Attorney - Employers should download this form and fill it out so that Summit Payroll can discuss matters with the EDD regarding tax payments and filings and so that Summit Payroll can file returns on behalf of the client
